What, in the name of all that's holy, can this blather mean?
How am I supposed to be able to "specify the duration" of my consent?
"The following statement applies," except there isn't any following statement.
And my favorite, "if we obtain your signature on this form by conditioning our services on your consent, your consent will not be valid. . . . we need your permission to . . . see if you are eligible for etc. etc." Doesn't that mean they are conditioning their services on my consent? (And why in the world do they need to check my age, state of residence, et cetera. They've already got all that information.)
